Family Choices is an Ageing, Disability and Home Care (ADHC) funded program which is managed by CareSouth and supported by its consortium partners; Anglicare in the Southern regions of NSW and Care West in the Western regions of NSW.
The Familly Choices program provides voluntary alternative family based care for children and young people with disabilities who are unable to be cared for in their own home, due to a range of circumstances. Priority is given to children 12 years and under. A number of voluntary, long-term care options are available including; foster care, shared care or relative/kinship care.
CareSouth has a proven record of effectively and compassionately working with children with disabilities. We make decisions based on what is in the child’s best interests. We actively assist children and families to participate in decision making regarding their lives.
CareSouth’s role
CareSouth coordinates the Family Choices program. It receives referrals from a ADHC panel, recruits, assesses, trains and matches appropriate alternate families with the child or young person and their family. CareSouth coordinates the development of a case plan and Alternative Family Care Agreement and coordinates, monitors and reviews all placements.
