Support coordination at CareSouth enables you to manage and direct your services. It aims to help you find and link in with community, mainstream or government options that are right for you. Our Support Coordinators will help you get all your supports up and running – assisting you to make the most of your plan and its flexibility. Specifically this includes help with:

• Planning and goal setting – working towards the goals set out in your NDIS plan
• Coordinating supports and assisting you to participate in your community
• Making sure services meet their obligations (including funded NDIS services as well as housing, education, justice and health)
• Supporting you to build your capacity to self-direct services and supports in the longer term
• Providing support and assistance for you in times of crisis
• Providing the NDIA with reports on outcomes achieved through your plan
